The Lungs and the Mechanics of Breathing

Overview
The lungs are a pair of spongy organs in the thoracic cavity whose central job is gas exchange: bringing oxygen into the blood and removing carbon dioxide.
Structure
Air travels through a branching system of airways:
- Trachea → bronchi → bronchioles → alveoli.
The right lung has three lobes, while the left lung has two, leaving room for the heart in the cardiac notch. Each lung is wrapped in a double-layered membrane called the pleura, whose thin fluid film lets the lungs glide smoothly during breathing.
The Alveoli
At the end of the smallest airways lie hundreds of millions of alveoli — tiny, thin-walled sacs surrounded by capillaries. Their enormous combined surface area makes efficient gas exchange possible.
Mechanics of Breathing
Breathing is driven by pressure differences:
- Inhalation: The diaphragm contracts and flattens, the rib cage expands, thoracic volume increases, and pressure drops, so air flows in.
- Exhalation: The diaphragm relaxes and rises, thoracic volume decreases, pressure rises, and air flows out. At rest, exhalation is largely passive.
Clinical Significance
Conditions such as pneumonia (fluid in alveoli), asthma (narrowed bronchioles), and pneumothorax (air in the pleural space) each disrupt a specific part of this system, which is why understanding the normal anatomy is essential.
Summary
The lungs combine an enormous alveolar surface with a pressure-driven pump powered by the diaphragm. Every breath is a controlled cycle of volume and pressure change.
