The Meninges: Protective Layers of the Brain and Spinal Cord

Overview
The meninges are three connective-tissue membranes that wrap the brain and spinal cord, providing protection and support. Between and beneath these layers lie spaces that are central to understanding head injuries and infections.
The Three Layers
From outside in:
- Dura mater: The tough, thick outer layer that lines the inside of the skull.
- Arachnoid mater: A thin, web-like middle layer.
- Pia mater: A delicate inner layer that clings tightly to the surface of the brain, following every fold.
The Key Spaces
- Subarachnoid space: Lies between the arachnoid and pia, filled with cerebrospinal fluid (CSF) that cushions the brain.
- Potential spaces around the dura become important in bleeding.
Cerebrospinal Fluid
CSF circulates in the subarachnoid space and the brain's ventricles, supporting the brain's weight, cushioning impacts, and removing waste. A lumbar puncture samples this fluid from the lower spinal canal to help diagnose infections and other conditions.
Clinical Significance
Bleeding between the meningeal layers produces distinct patterns:
- Epidural hematoma: between skull and dura, often from arterial injury.
- Subdural hematoma: beneath the dura, usually from torn veins.
- Subarachnoid hemorrhage: into the CSF space, often from a ruptured aneurysm.
Infection of the meninges is called meningitis, a potentially serious condition.
Summary
The three meninges and the fluid-filled subarachnoid space protect and cushion the central nervous system. Knowing these layers explains the different types of intracranial bleeding and the basis of a lumbar puncture.
