The Diaphragm: The Main Muscle of Breathing

Overview
The diaphragm is a dome-shaped sheet of muscle and tendon that separates the thoracic cavity above from the abdominal cavity below. It is the principal muscle of breathing, responsible for most of the air movement during quiet respiration.
Structure
The muscle fibers arise from the lower ribs, the sternum, and the lumbar vertebrae, and converge on a central tendon at the top of the dome. Because it attaches all around the lower margin of the rib cage, its contraction pulls the dome downward and flattens it.
Major Openings
Several structures must pass between the chest and abdomen, so the diaphragm has three main openings:
- Caval opening: transmits the inferior vena cava.
- Esophageal opening: transmits the esophagus and vagus nerves.
- Aortic opening: transmits the aorta, along with the thoracic duct.
Role in Breathing
When the diaphragm contracts, it descends, increasing the volume of the thoracic cavity and drawing air into the lungs. When it relaxes, it rises back into its dome shape, helping push air out. It is controlled by the phrenic nerve, which arises from cervical roots C3–C5.
Clinical Significance
Damage to the phrenic nerve paralyzes one half of the diaphragm, impairing breathing. A weakness in the diaphragm at the esophageal opening can allow part of the stomach to push upward, producing a hiatal hernia.
Summary
The diaphragm is the engine of respiration: its downward contraction expands the chest and drives inhalation, while precise openings allow vital structures to pass between the two body cavities.
