Overview

The heart does not rely on the brain to beat. It contains specialized muscle cells that generate and conduct electrical impulses, producing a coordinated, rhythmic contraction. This is the cardiac conduction system.

The Pathway of an Impulse

  1. Sinoatrial (SA) node: Located in the right atrium, it is the heart's natural pacemaker, setting the pace at roughly 60–100 beats per minute at rest.
  2. Atrioventricular (AV) node: Sits between the atria and ventricles. It briefly delays the impulse, allowing the atria to finish emptying before the ventricles contract.
  3. Bundle of His: Carries the impulse into the interventricular septum.
  4. Right and left bundle branches: Distribute the signal down each side of the septum.
  5. Purkinje fibers: Spread the impulse rapidly through the ventricular walls, triggering a strong, unified contraction.

Why the Delay Matters

The AV node's pause is essential. Without it, atria and ventricles would contract at the same time, and the ventricles would not fill properly. This sequencing is what makes the heart an efficient pump.

Clinical Significance

Disruptions in this system cause arrhythmias. If the SA node fails, lower pacemakers can take over but at a slower rate. Blocks in the bundle branches alter the heart's electrical pattern, which is exactly what an electrocardiogram (ECG) is designed to detect.

Summary

The conduction system turns the heart into a self-regulating pump: the SA node sets the rhythm, the AV node times the sequence, and the His-Purkinje network delivers a coordinated ventricular contraction.
