The Large Intestine: Water, Storage, and the Microbiome

Overview
The large intestine is the final section of the digestive tract. It is shorter but wider than the small intestine, and its main jobs are to absorb water and electrolytes, compact waste into stool, and house the trillions of bacteria of the gut microbiome.
Parts
- Cecum: A pouch at the start, with the appendix attached.
- Colon: Divided into ascending, transverse, descending, and sigmoid portions that frame the abdomen.
- Rectum: Stores stool before elimination.
- Anal canal: The exit, controlled by internal and external sphincters.
Distinctive Features
The wall of the colon shows three features not seen in the small intestine: bands of muscle called teniae coli, pouches called haustra, and small fat-filled tags. These give the large intestine its characteristic segmented appearance.
Functions
- Water absorption: Reclaiming water turns liquid contents into solid stool.
- Electrolyte balance: Sodium and other ions are absorbed here.
- Microbiome support: Gut bacteria ferment undigested fiber, producing some vitamins (such as vitamin K) and beneficial compounds.
Clinical Significance
Inflammation of the appendix (appendicitis) is a common surgical emergency. The colon is also the most frequent site of colorectal cancer, which is why screening for it is widely recommended.
Summary
The large intestine finishes the digestive process by absorbing water, forming and storing stool, and supporting a vast community of helpful bacteria. Its segmented structure and microbiome set it apart from the rest of the gut.
