The Pancreas: A Dual-Function Gland

Overview

The pancreas is a soft, elongated gland lying behind the stomach, stretching across the back of the abdomen. It is unusual because it acts as two glands in one: an exocrine gland for digestion and an endocrine gland for blood sugar control.

Anatomy

The pancreas has four parts: the head (nestled in the curve of the duodenum), the neck, the body, and the tail (reaching toward the spleen). Its main duct carries digestive juice into the duodenum, usually joining the bile duct just before the entry point.

Exocrine Function

Most of the pancreas is exocrine tissue that produces digestive enzymes to break down proteins, fats, and carbohydrates. These enzymes flow through the pancreatic duct into the small intestine, where they do the bulk of chemical digestion.

Endocrine Function

Scattered through the gland are clusters of cells called the islets of Langerhans. They release hormones directly into the blood:

  • Insulin, which lowers blood sugar.
  • Glucagon, which raises it.

Together these hormones keep blood glucose within a narrow range.

Clinical Significance

Loss of insulin-producing cells leads to diabetes mellitus. Inflammation of the gland (pancreatitis) can be severe because activated enzymes may begin to digest the pancreas itself. Because the pancreas lies deep in the abdomen, tumors often cause symptoms only at a late stage.

Summary

The pancreas is two glands in one: it pours enzymes into the gut to digest food and releases insulin and glucagon into the blood to control sugar. Its deep position and powerful enzymes shape its most important diseases.
